Welcome to Cerevia. These Terms of Service (“Terms”) govern your access to and use of our website, assessment, therapy, coaching, behavioral support services, and related offerings. By visiting our site, scheduling appointments, or using our services, you agree to comply with these Terms.
1. Services
Cerevia provides assessment, therapy, coaching, autism support, ADHD support, behavioral support, and related wellness services for children, teens, adults, and caregivers.
All services are subject to availability, scheduling, and professional suitability. We reserve the right to modify or discontinue services without prior notice.
You agree to provide accurate contact, billing, and relevant service information when booking appointments or communicating with our team.
Appointments and service plans are intended for the individual or family for whom they are booked and are non-transferable unless approved by Cerevia.
2. Appointments & Participation
Clients and caregivers are expected to attend scheduled appointments on time and participate respectfully in the therapeutic process.
For child and family services, caregiver involvement may be recommended or required to support progress and continuity of care.
Cerevia reserves the right to refuse or discontinue services in cases of unsafe behavior, harassment, repeated missed appointments, or violation of these Terms.
Recommendations, strategies, and treatment plans are personalized and should be followed as discussed with your provider.
3. Health & Safety
Our services are intended to support emotional, behavioral, developmental, and mental wellness, but they do not replace emergency medical or psychiatric care.
If you or someone in your care is experiencing a crisis or emergency, please contact emergency services or an appropriate crisis support provider immediately.
Clients and caregivers are responsible for providing accurate health, developmental, and relevant background information to support safe and effective care.
Cerevia and its providers are not liable for outcomes resulting from incomplete information, failure to follow recommendations, or circumstances beyond our reasonable control.
4. Payments & Cancellations
Payment for services is due at the time of booking, at the appointment, or as otherwise outlined in your invoice or service agreement.
Appointments may be canceled or rescheduled with at least 24 hours notice. Late cancellations or missed appointments may be subject to cancellation fees.
Refunds for prepaid services or programs are subject to Cerevia’s cancellation and refund policy. Promotional offers, packages, and discounts may have specific terms and conditions.
Outstanding balances may need to be resolved before future appointments can be scheduled.
5. Website Use & Content
All content on the Cerevia website, including text, graphics, resources, program materials, and service descriptions, is owned by Cerevia or used with permission.
You may not copy, reproduce, distribute, modify, or use our content for commercial purposes without prior written consent.
Users agree not to misuse the website, attempt unauthorized access, or interfere with its functionality or security.
6. Changes to Liability
Cerevia provides professional support services, but individual results may vary based on personal circumstances, participation, and external factors.
We are not liable for indirect, incidental, consequential, or special damages arising from the use of our website or services.
Liability for any claim related to our services is limited to the amount paid for the specific service giving rise to the claim, to the extent permitted by law.
